Creating Forms
ProForma provides a couple of pre-made templates you can use to begin exploring. There are templates for every team, such as Finance, IT, and Marketing.
If you’d like to create your own form you can go to the ProForma admin settings (Settings → Add-Ons → Proforma → Forms). Here you can customize the form to have sections, fields inside each section, make these fields required, and so much more! In addition you can link a form to a specific project and/or issue type. Better yet, you can link it to multiple projects or issue types!

- If you will be using this with Jira Service Desk know that you can mark the form as external, will display in the service desk portal, or keep it only internal, will show only inside the Jira issue.
- You can decide whether you want to lock the form after submission. The user will only be able to download a PDF if the form is locked and submitted.
- Automate when to add a form. For example, if you want to have a form added when an issue transitions from Open → In Progress.
